Dra. María Noelia Vivacqua cotrópicas. Esto es especialmente cierto para el trastorno depresivo mayor. Una revisión advierte que aún en los casos en que la asociación entre el uso de farmacogenómica y la obtención de mejores resultados en los tratamientos de TDM son comprobables existe un riesgo que la magnitud de esta asociación sea “inflada” (31). La interacción entre genes diferentes, y entre genes y moduladores ambientales deben ser tenidos en cuenta, porque pueden llevar a resultados ambiguos en el análisis para interpretar el rol de la variabilidad genética en la respuesta a psicofármacos. La mayoría de los antidepresivos son metabolizados por citocromo P450 isoenzimas 2D6 y 2C19. El análisis de polimorfismos para los genes que codifican estas enzimas junto con otros de transportadores de drogas o receptores específicos demuestra, cada vez más, una asociación a la respuesta de la droga y a efectos adversos en el contexto de un trastorno mayor de salud mental. Sin embargo, su interpretación puede ser muy dificultosa en el contexto de enfermedades de la salud mental, en el que el resultado clínico final depende de múltiples y diversas variables. Un posible enfoque puede ser utilizar la información farmacogenómica como una herramienta para realizar una elección farmacológica sin dejar de lado otros factores que influyen en la elección de los psicofármacos. Los polimorfismos genéticos representan el marcador biológico más prometedor y fácilmente accesible de la respuesta a antidepresivos (21). La implementación de la investigación del genoma en la práctica clínica habitual podría mejorar la experiencia terapéutica en el tratamiento del TMD. Más investigaciones deben ser llevadas a cabo para desarrollar lo que impresiona el gran potencial de las técnicas farmacogenómicas en psiquiatría. Agradecimientos Dr. Waldo Belloso Dra.
