Conversaciones entre el Sistema Nervioso y el Sistema Inmune. A propósito del “estrés” y de las “enfermedades autoinmunes”. Talks between the Nervous System and the Immune system. About ¨stress¨and ¨autoimmune diseases¨. Resumen En los últimos años, con la profundización del conocimiento de las comunicaciones celulares, se ha observado un notable diálogo entre el sistema inmune y el sistema nervioso. La posibilidad actual de lograr un mayor acercamiento a la comprensión de la fisiología de los dos sistemas en cuestión, permite entender la comunicación entre ellos y también con el sistema endócrino. El estrés, visto desde el punto de vista psicológico, ha demostrado a través de numerosos estudios, inducir y modificar determinadas respuestas del sistema inmune. Esto es posible principalmente a través de la acción sobre el sistema HPA (hipotálamo-pituitario-adrenal) y el eje SMA (simpático-médula adrenal). Su acción se hace ostensible a través de una desregulación inmunológica que lleva a efectos deletéreos sobre la evolución o predisposición a enfermedades infecciosas, alteraciones en la inmunización con vacunas, trastornos en la cicatrización de heridas, reagudización o aparición de enfermedades autoinmunes y posiblemente de tumores. La relación directa “sistema nervioso-enfermedades autoinmunes” es ampliamente conocida desde hace años, a través de las neurohormonas del eje HPA. En esta presentación nos abocaremos al efecto directo del Sistema Nervioso Central (SNC) y del Sistema Nervioso Simpático (SNS) sobre las enfermedades autoinmunes, tema menos conocido que se tratará de clarificar a través de los modelos animales y ejemplos clínicos. En primer lugar se realizará un breve recordatorio de los elementos básicos y la fisiología del sistema inmune para poder abocarnos, posteriormente, a una visión central respecto de los dos temas mencionados: “El estrés y la inmunomodulación”, y la “Interacción del sistema nervioso y las enfermedades autoinmunes”. Abstract In the last years, a notorious dialogue between the immune and nervous systems was observed thanks to the improvement of the knowledge about cellular communications. Nowadays it is possible to manage a closer approach to the comprehension of both systems leading to a better understanding of the communication between them and with the endocrine system as well. From a physiological point of view, several studies have shown that stress induces and modifies specific answers of the immune system through its actions on the HPA system (Hypothalamus-pituitary-adrenal gland) and on the SAM axis (sympathetic-adrenal medulla). This action is more notorious through an immunological dysregulation which leads to deleterious evolution or predisposition to infectious diseases, alterations in vaccine induced immunization, wound healing disorders, relapse or appearence of autoimmune diseases and tumors. This subject will be reviewed in the first part of the present article. The second topic of this article, -the direct relation “nervous system-autoimmune diseases”-, the connection between that relation and the neurohormones of the HPA axis has been thoroughly known for years; that is why we shall concentrate in the less known theme, the direct effect of the Central Nervous System (CNS) and the Sympathetic Nervous Sytem (SNS) on autoimmune diseases which will be clarified with animal models and clinical examples. As certain concepts of immunology are far from the Psychiatry and the Psychopharmacology, a brief review of the basic elements and physiology of the immune system will be performed, and later a zenithal vision of the two mentioned themes will be performed: “stress and immunomodulation”, and the “Interaction between the Nervous system and autoimmune diseases”.
