Actualización del manejo terapéutico del Trastorno Obsesivo Compulsivo

Psicofarmacología

Psicofarmacología 6:37, Abril 2006 la conciencia en forma de obsesiones, y/o favorecer la selección de conductas que podrían manifestarse en forma de actos compulsivos. En el TOC, existiría una alteración en la regulación de la activación-inhibición a nivel subcortical y el tono de la vía directa estaría aumentado con respecto al indirecto. Esto produciría la activación de la corteza y el tálamo lo que amplificaría y perpetuaría el proceso, a partir de lo cual se generaría un circuito auto reverberante. Por su parte, el tono indirecto resultaría insuficiente para cortar o limitar dicho circuito, situación que se ha visto que revierte con el tratamiento adecuado, reestableciendo el equilibrio funcional entre ambas vías. Se han realizado múltiples trabajos clínicos que demostraron la eficacia de los ISRS en el tratamiento del TOC, y constituyen los fármacos de primera elección. Los ISRS producirían modificaciones en los receptores específicos ubicados a nivel basal, restableciendo el tono en ambas vías. Existe un porcentaje considerable de pacientes que no responden a los tratamientos convencionales establecidos y las alternativas coadyuvantes propuestas no son del todo concluyentes. Los mecanismos hasta aquí explicados, solo muestran parte de lo conocido actualmente y el TOC continúa siendo un desafío tanto para los investigadores y para los profesionales que tratan el TOC, así como para aquellos que lo padecen.
