Universidad de Buenos Aires Resumen Los pacientes diabéticos son considerados según las últimas guías de evaluación del riesgo cardiovascular como pacientes que presentan “muy alto riesgo”. La presencia de LDLc como partículas pequeñas y densas es uno de los mecanismos principales que explican el mayor riesgo aterogénico. También lo es el incremento de las partículas lipoproteicas remanentes. Se analiza en el artículo porqué la administración de estatinas de alta intensidad, en general con inhibidores de la absorción de colesterol es esencial en este tipo de pacientes. Abstract According to the latest cardiovascular risk guidelines, diabetic patients are considered to be at a “very high cardiovascular risk”. The presence of LDL-C as small and dense particles, as well as the increase in the remnant lipoprotein particles, are two of the main mechanisms which account for the highest atherogenic risk. This article seeks to analyze the reason why the administration of highintensity statins, usually in conjunction with cholesterol absorption inhibitors, is key among these types of patients. La prevalencia mundial de la diabetes mellitus se calculaba en 2011 en 366 millones de personas y se pronostica que esa cifra llegue a 552 millones en el 2030. En nuestro país, aproximadamente el 9,5% de la población padece esta enfermedad y si agregamos pacientes portadores de síndrome metabólico fundamentalmente por la alta incidencia de hipertensión arterial, obesidad y dislipidemia la cifra supera holgadamente el 35% de nuestros habitantes. En este artículo desarrollaremos la fisiopatología y el tratamiento de la llamada forma mixta de la dislipidemia aterogénica del síndrome metabólico (SM) y la diabetes (DBT) que constituye un elemento fundamental en la progresión de las complicaciones macrovasculares de estos pacientes. Las guías de tratamiento de los lípidos tanto el ATP III como las del 2014 conjuntas del American College of Cardiology y del American Heart Association ya fueron descriptas en detalle en otras ediciones de Farmacología Cardiovascular por lo que no abundaremos en su descripción. Solo quisiéramos destacar que las guías del ATP III habían ya calificado al paciente diabético por el simple hecho de serlo, como de “alto riesgo” equiparándolo al paciente no diabético que había sufrido un evento cardiovascular (CV) fue llamado equivalente a enfermedad coronaria. La ampliación del 2004 incluye a los diabéticos en una nueva categoría de “muy alto riesgo” cuando ya habían presentado un evento CV llevando la meta de descenso de su colesterol LDL a menos de 70 mg / dl. También se agregó en ese grupo a los pacientes con SM con triglicéridos altos (> 250mg / dl) y HDL-c bajo (
