Andrea Inés Taboada UNA REVISIÓN DE LA PATOGÉNESIS Y TERAPÉUTICA FARMACOLÓGICA DE LA ENFERMEDAD DE ALZHEIMER A Review of the pathogenesis and pharmacological therapeutics of Alzheimer´s disease Fecha de recepción: 8 de Agosto de 2005 // Fecha de aceptación: 24 de Octubre de 2005 Resumen Con relación al tratamiento farmacológico de la enfermedad de Alzheimer (EA), cuatro drogas inhibidoras de la acetilcolinesterasa (AChE-I) (tacrina, rivastigmina, donepecilo y galantamina) han sido aprobadas por las autoridades sanitarias de varios países desarrollados para su comercialización,sobre la base de la demostración de efectos terapéuticos leves en las pruebas cognitivas y de funcionalidad general. El criterio subyacente es que el beneficio obtenido, aunque muy modesto, es superior a la falta absoluta de eficacia de otras modalidades terapéuticas ensayadas en el pasado. Sin embargo, en la actualidad, esta interpretación se está revisando a la luz de los notables avances en la comprensión de la fisiopatología de este padecimiento. El paradigma original, hoy cuestionado, consistía en la falta de neurotransmisión colinérgica como manifestación de la enfermedad. Existen numerosos datos consistentes con la hipótesis colinérgica, aunque el balance riesgo / beneficio nunca fue satisfactorio. El cuestionamiento actual deriva de analizar el mecanismo mediante el cual el SNC se adapta, especialmente en las etapas iniciales de la enfermedad, a la agresión que culmina con la muerte neuronal. Hoy contamos con una creciente certeza de que la actividad de la acetilcolinesterasa (AChE) es necesaria para una efectiva, aunque transitoria, compensación. En este contexto, el empleo de anticolinesterásicos debería, paradójicamente, empeorar el curso del trastorno, posibilidad que no ha sido aún efectivamente indagada a través de ensayos clínicos. La concepción dominante a la fecha incluye a la enfermedad de Alzheimer entre las afecciones neurodegenerativas inducidas por defectos en el plegamiento proteico. Abstract With regard to the pharmacological treatment of Alzheimer´s Disease (AD), four acetylcholinesterase inhibitors (AchE-I) (tacrine, rivastigmine, donepezil and galantamine) have been approved for commercialization by the sanitary authorities of a wide range of developing countries on the basis of the slight therapeutic effects displayed in cognitive and general functionality tests. The underlying criterium is that the obtained benefit, though very slight, supersedes the complete lack of efficacy of other forms of therapy conducted in the past. However, nowadays, this perspective is being revised in the light of remarkable improvements in the comprehension of the physiopathology of this disease. The original paradigm, which is currently questioned, consisted in the lack of a cholinergic neurotransmission as the manifestation of the disease. There is a great amount of data consistent with the cholinergic hypothesis, although the riskbenefit balance has never been satisfactory. The current question results from analyzing the mechanism by which the central nervous system (CNS) adapts, especially during the first stages of the disease, to the aggression that ends in neural death. Nowadays, we have a strong conviction that the acethylcholinesterase (AChE) action is necessary for an effective, though temporary compensation. Within this context, the use of anticholinesterases should paradoxically worsen the development of this disorder, a possibility which has not yet been delved into through the conduction of clinical essays. The prevailing conception to this date encompasses Alzheimer´s Disease among the neurodegenerative afflictions induced by defects in the protein aggregation.
